The following is not intended to replace the campus evaluations completed at the end of the semester. They are provided simply as a means of communicating problems and/or praises that should be brought to instructor's attention during the semester.  Please select the desired form of communication.

• Send e-mail to the course coordinator.
• Print a standard comment form and place it anonymously in the instructor's mailbox in IDE G1.
